    Esperance Home Care is a non-profit community organisation supported by the Shire of Esperance. Out team of professional staff and volunteers provide key support services to frail older people, young and aged people with a disability and their carers. We aim to help people continue to live comfortably in their own home and to prevent people prematurely entering residential aged care. Esperance Home Care provides a range of services suited to the needs of individuals and their carers. These include areas such as: - Personal care - Domestic Assistance - Home Maintenance - Carer Support and Respite - High needs service - Transport - Meals on Wheels --Programmes- Esperance Home Care provides an extensive range of programmes and outings for both men and women. The programmes provide activities that are suitable for frail older people and people living with a disability. These include: - Programmes specifically designed for men - Dementia specific programmes - Group activities - Activities to increase skills and knowledge - Opportunities for day trips and outings. You can access a full list of programmes from our website or by contacting Home Care administration. --Adult Day Care-- The Day Centre provides respite for frail older people and younger persons living with a disability. Transport is provided by our wheelchair accessible bus which will pick you up from your home and drop you back again. Join in our activities programme and enjoy sharing your time with other people as you catch up over coffee, go on outings or just be involved. The centre is staffed by friendly professional people trained to meet the needs of those who attend. Staff are also assisted by volunteers who help in a variety of areas. --Eligibility & Assessment-- People who want to apply for support must meet eligibility criteria for the services through a confidential assessment process that will take into consideration your special needs.
