Home Visits Onsite Workplace Physiotherapy - Preventive and injury management programs Motor vehicle accidents Costs Flex Out Physiotherapy practices are connected to HICAPS so patients are able to claim directly through their health funds. We accept cash, cheque, EFTPOS, Visa and Mastercard. Patients on LMO approved Medicare Enhanced Primary Care (EPC) Program and DVA are bulk billed directly, and workcover claims are invoiced directly to the appropriate third party. Referral requirements Private consultations - No referral is necessary Workers compensation- referral necessary including area of body to be treated Department of Veteran Affairs (DVA) - D904 referral form required (valid for 12 months) Medicare EPC Program - Patients with an EPC multidisciplinary care plan can be referred by their GP for up to a maximum of 5 allied health visits per year. These patients require a Referral Form for Allied Health Services under Medicare .
